一、依據教育部 115 年 1 月 28 日臺教師(一)字第 1152600261 號函辦理。

二、藝術教育貢獻獎績優學校獎由學校向本局申請；績優團體之推薦，由各團體、法人列舉具體事實及證明，並以書面向本局推薦；個人獎項之推薦由學校、團體於各單位內進行實質審核並推薦後，由學校或團體列舉具體事實及證明，並以書面向本局推薦，申請方式如下：

(一)報名期間：即日起至 5 月 31 日止。

(二)繳交資料：請於 115 年 5 月 31 日前，將推薦表、佐證資料及光碟以郵寄或親送至「臺南市六甲區林鳳國民小學」（地址： 734 臺南市六甲區中社里 13 鄰林鳳營 240 號）並註明參加「臺南市藝術教育貢獻獎」，郵戳為憑，逾期恕不受理，電話： 06-6983583 分機 123 。

１、推薦表：請推薦單位於「推薦意見」欄加註評語及核章（如為學校單位請蓋學校關防）。

２、顯著事蹟：應依事蹟發生先後，詳實敘明。

３、佐證資料：請擇要精簡敘述，以 A4 紙裝訂成冊(每冊限 200 頁，最多以 2 冊為限)，並加製封面與目錄， 1 式 1 份。內容得為獎狀、教材教案、書籍、媒體報導、影像資料等。

４、光碟：包括推薦表（含 word 檔及用印後之 pdf 檔）、佐證資料電子檔及活動照片 6 至 10 張(需標註照片名稱，圖片大小至少 1Mb)。

三、初選公布後，本局委託承辦學校協助獲選學校補正文件，於 115 年 6 月 30 日前，將初選結果連同相關資料報教育部進行決選。
