說明：

一、依據臺南市政府衛生局 115 年 1 月 9 日南市衛心字第 1150003069 號函辦理。

二、該局與國立成功大學醫學院附設醫院、奇美醫療財團法人奇美醫院（含樹林院區）、衛生福利部嘉南療養院、奇美醫療財團法人柳營奇美醫院等醫療機構合作辦理酒癮戒治處遇治療業務，若遇個案欲使用酒癮補助戒酒者，可輔導個案填寫「臺南市政府衛生局 115 年度酒癮治療費用補助方案轉介單」（如附件 1 ），並傳真至該局（ 06 ） 3358161 或 E-mail ： b00006@tncghb.gov.tw ，後續由該局轉請醫院安排治療。

三、旨案每人每年累計補助額度以 4 萬元為限，補助時間： 115 年 1 月 1 日起至年度「酒癮治療服務方案」計畫補助經費用罄（公務預算與家防基金合併計算）。

四、另酒駕防制教育及酒癮治療實施管理辦法之酒癮治療（酒駕吊銷執照須重考駕照者），其治療費用為完全自費且須由所轄監理站所申請轉介單，至國立成功大學醫學院附設醫院、奇美醫療財團法人奇美醫院（含樹林院區）、衛生福利部嘉南療養院等三家機構接受治療。

五、檢附「臺南市政府衛生局 115 年度酒癮治療費用補助方案轉介單」（附件 1 ）及「酒癮治療費用補助方案說明書」（附件 2 ），相關酒癮戒治資訊可至該局官網 https://reurl.cc/jmor5Z 查詢，或致電該局酒癮戒治防治專線（ 06 ） 2679751 轉 176 詢問。
